  1. Samples shall be accepted only with proper name plate/marking label, wherever applicable.
  2. I have been informed that the latest applicable test/calibration method/protocol would be followed, unless specified.
  3. Testing/calibration of the sample shall be initiated after submission of documents and payment.
  4. All possible care shall be taken in handling the sample, but the risk of damage in transit or during testing/calibration rests with the customer only. BTH shall not be responsible for any damage to the samples during loading, unloading, shifting and testing, or due to acts of God like fire, accidents, storms, earthquakes etc.
  5. BTH shall not be responsible if a test/calibration has to be postponed because of non-availability of power, malfunctioning of the equipment etc.
  6. BTH can retain the sample as per the retention period, except for perishable items. Tested samples shall be disposed of by BTH after 30 days from the date of issue of the test report, or as per the guidelines of regulatory authorities. The customer shall collect the sample within 7 days after the retention period. BTH shall not be held responsible for disposal after the said period.
  7. The technical details mentioned above are correct to the best of my knowledge and belief. I abide by the same and understand its importance. I fully agree that there will be no change whatsoever from our side, and request BTH to proceed with testing/calibration.
  8. If any additional test(s)/calibration is required by the customer after the request is processed, the same would be entertained only if sufficient sample is available/feasible, and will be mentioned in the report after proper contract review. Charges would be extra.
  9. In case of any dispute, the decision of the Director, BTHPL shall be final.
